By using lumeraagelesscare.us or booking services with Lumera Wellness LLC, you agree to these terms.

Services

Lumera Wellness provides aesthetic and wellness services performed by a licensed Registered Nurse under physician-delegated protocols. Information on this site is general in nature and is not medical advice. It does not create a provider-patient relationship. All treatments require an in-person evaluation and written informed consent.

No guarantee of results

Individual results vary. No specific outcome is promised or guaranteed. Fees are for services rendered, not for a particular result.

Appointments, deposits, and cancellations

A deposit may be required to reserve certain appointments and is applied to the cost of your treatment. We ask for 24 hours' notice to cancel or reschedule. Cancellations within 24 hours and missed appointments forfeit the deposit. Arriving more than 15 minutes late may require shortening or rescheduling your appointment.

Payment

Payment is due at the time of service. Packages and prepaid series are non-refundable and non-transferable unless otherwise stated in writing.

Text messaging

If you provide your mobile number and consent, you may receive appointment reminders, treatment follow-ups, and account notifications by text. Message and data rates may apply. Message frequency varies. Reply STOP to opt out or HELP for help. Consent to receive text messages is not a condition of purchase.

Eligibility

You must be at least 18 years of age to book or receive services.

Limitation of liability

To the fullest extent permitted by law, Lumera Wellness LLC is not liable for indirect or consequential damages arising from use of this website.
