Lumera Wellness LLC ("we," "us," or "our") operates lumeraagelesscare.us. This policy explains how we collect, use, and protect your information.
We collect information you provide directly to us, including your name, email address, phone number, date of birth, and health information relevant to the services you are seeking. We collect this when you book an appointment, complete intake forms, contact us, or sign up for communications. We also collect limited technical information such as browser type and pages visited.
We use your information to schedule and provide services, send appointment reminders and confirmations, follow up after treatment, respond to your questions, process payments, maintain your medical record as required by law, and — where you have separately agreed — send you information about our services.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
If you provide your mobile number and consent to receive text messages, you may receive appointment reminders, treatment follow-ups, and account notifications from Lumera Wellness. Message and data rates may apply. Message frequency varies. You may opt out at any time by replying STOP. Reply HELP for assistance, or contact us at results@lumeraagelesscare.us or (720) 897-3774.
We do not sell your personal information. We share information only with service providers who help us operate the practice — such as our electronic medical record system, payment processor, and scheduling software — and only to the extent necessary for them to perform those services. We may disclose information where required by law or to protect health and safety.
Health information you provide is treated as confidential and handled in accordance with applicable privacy laws, including HIPAA where it applies to our services. Our Notice of Privacy Practices is available on request and provided at your first visit.
We use commercially reasonable safeguards to protect your information, including encrypted storage and access controls. No method of transmission over the internet is completely secure, and we cannot guarantee absolute security.
You may request access to, correction of, or deletion of your personal information by contacting us. You may opt out of marketing communications at any time. Opting out of marketing does not affect appointment-related messages necessary for your care.
Our services are intended for individuals 18 years of age and older. We do not knowingly collect information from anyone under 18.
We may update this policy from time to time. The effective date above reflects the most recent revision.
